Can anyone explain what gets a file added to the "Open Recent" list? Sometimes files I open appear on the list, and sometimes they don't.

I think it's just files opened from the File/open dialogue or double click opened from Windows Explorer. If you open a part from within the assembly, or open a part/assy from a drawing, it does not include it in Recent.
